IN BRIEF

The award recognises the care-integration project; it does not create a public portal or change a patient's clinical treatment by itself. People should continue to use their normal health and social-service contacts.

  • The system has linked social information across hospitals and health centres since 2016.
  • The award includes €4,000 for training in integrated care.
  • Social-referral and clinical-discharge monitoring functions are being implemented or developed.
01

The system supports professionals across a care pathway

The record replaced handwritten social-service files with a shared digital process, allowing authorised teams to see relevant social information as a patient moves between settings. SESARAM says this reduces duplicated information and administrative delay.

Access remains professional and purpose-bound. The award is not permission for the public or unrelated staff to view a patient's social record.

02

An award is evidence of recognition, not a service guarantee

The Integrated Care Award was announced at a national meeting and recognises the project's approach. It does not independently prove that every discharge or referral is timely or error-free.

A patient with a care-coordination concern should contact the responsible SESARAM service. Urgent medical needs still follow the established health and emergency channels.

WHAT TO DO NEXT

What you can do now.

  1. 01

Use the normal SESARAM contact for an individual case.

  2. 02

Ask which team owns a pending social referral or discharge issue.

  3. 03

Use 112 for an immediate medical emergency.
